3D scan of a baby’s head captured with Forma

Forma — 3D Cranial Scanning

Capture a baby’s head shape in 3D on an iPhone — radiation-free — and send it securely to Osteo3d to help craft a corrective cranial helmet.

For clinicians & orthotists

A safer, faster way to fit cranial helmets

For infants with plagiocephaly or brachycephaly, a corrective helmet is shaped to the head. Forma offers a quick, contactless way to capture the head shape in 3D — taken in clinic on an iPhone and sent securely to Osteo3d.

How it works

From scan to helmet in four steps

1

We issue a Case ID + PIN

Osteo3d creates a secure case for each patient — no patient name is ever stored in the app.

2

Scan the head in 3D

On a LiDAR iPhone, the clinician captures a to-scale 3D scan of the head in under a minute, with on-screen guidance.

3

Secure upload to Osteo3d

The scan uploads straight to Osteo3d — the file never leaves the device any other way.

4

We craft the helmet

Our team turns the scan into a precise corrective cranial helmet.

Why Forma

Built for clinical use

  • Radiation-free 3D capture

Uses the iPhone’s LiDAR to capture the head’s shape in 3D for helmet design — with no ionising radiation.

  • Guided, one-minute capture

On-device guidance helps any clinician get a complete scan on the first try.

  • Privacy-first by design

A scan is identified only by its Case ID. No patient name is stored on the device.

  • Secure, upload-only

Scans go directly to Osteo3d; they can’t be shared or exported from the app.

  • Manufacturing-ready output

Produces to-scale OBJ/STL geometry our team uses to make the helmet.

Requirements

  • • iPhone Pro model with LiDAR (12 Pro or newer)
  • • iOS 17 or later
  • • During beta, installed via Apple TestFlight (we send the invite)

Privacy & consent

Each scan is identified only by a Case ID — no patient name is stored on the device. The app asks for consent before scanning, and data is used solely to manufacture the helmet. See our privacy policy.

Forma is an Osteo3d capture tool used under clinician oversight to support the design of custom cranial helmets. It is in limited evaluation and is not intended for diagnosis.
